License Procedure

Documents to be uploaded along with the application form for grant of License to Operate Blood Bank :-

1. Blue print with site plan of the premises duly signed by applicant/owner of premises & architect.

 2. Attested photocopy of copy of partnership deed
    or
   Memorandum and article of association, if it is Limited or Private Limited firm.

 3. Non conviction affidavits of Prop./partners/directors.

 4. Non conviction Affidavits of technical staff.

 5. Attested copies of qualification documents of all the technical staff and their approval/experience certificate.

 6. Copy of resolution of Board of Directors in favour of Authorised Signatory.

 7. Affidavits of authorized signatory.

 8. Photostat/self attested copies of academic qualification & approval of technical staff as per conditions lay down in Drugs & Cosmetics Act, 1940 & Rules, 1945.

 9. List of blood bank machinery and equipments.

10. List of technical staff duly signed by applicant.

11. Copy of Invoices/bills of machinery and equipments.

12. Copy of medical bio waste disposal agreements with company authorized by State Pollution Control Board.

13. NOC from fire department.

14. Affidavit of person responsible for day to day work and conduct & control of the business of the firm.

15. Ownership documents of the premises of Blood Bank.

16. Lease deed/rent agreement or rent receipt

17. List of voluntary donors.

18. Standard Operating Procedures

19. Land allotment papers

Food and Drug Administration Haryana Grants Blood banks License after approval from DCG(I), Central Licensing Authority (CLAA), New Delhi as per the provisions of Drugs Cosmetics Act-1940.

Procedure to obtain Blood Bank License (Fresh)

 

Stage1 
The Applicant has to apply online to the State Drugs Controller –cum- Licensing Authority of the State and the copy of application alongwith a requisite documents & with soft copy also forwarded to the DCG(I), New Delhi as well as CDSCO(NZ), Ghaziabad. The following documents have to be submitted to the State Drugs Controller, Haryana as well as DCG(I), New Delhi as well as CDSCO(NZ), Ghaziabad:-
The applicant has to make application in the requisite Form. The details of Forms and necessary requisite fees are given below:-

1.      Application form 27-C.

2.      Fees challan of Rs. 7500/-

3.      User can pay fee Online through Net Banking/Debit Card/Credit Card and can also download the challan and can pay the fees.


The fee can also be paid through challan at Government Treasury, under Head of Account- 0210 - Medical and Public Health. 04 - Public Health. 104 - Fees and Fines, etc.

Stage 2
The applicant will submit the application completed in all respect to the State Drugs Controller alongwith all relevant documents. Application is scrutinized and the application is forwarded to the CDSCO(NZ), Ghaziabad for joint inspection of the premises of the applicant firm. The premises is jointly inspected by the officers of CDSCO(NZ), Ghaziabad and the Senior Drugs Control Officer of the Concerned zone and the report is forwarded by Senior Drugs Control Officer along with his recommendation of the joint inspection team to State Drugs Controller. On the recommendation of the joint inspection team the case is sent to the DCG(I), New Delhi for approval of grant/renewal of license to operate Blood Bank.

Stage 3
Grant of Licence
On the receipt of the approval of the DCG(I), New Delhi the license is granted to the applicant blood bank by the State Drugs Controller and applicant gets information through SMS /email
 

Procedure to Renew Blood Bank License 
Licenses in Forms 27C be renewed.

Stage 1
Application for renewal of Blood Bank Licence


Documents to be uploaded and submitted along with application of renewal of licence


The applicant has to make application in the requisite Form 27C and pay Necessary fees as given in fee chart in Appendix A The mode of payment of fees is same as fresh licence.

 

User can pay fee Online through Net Banking/Debit Card/Credit Card and can also download the challan and can pay the fees.

 

1.      Self certified copy of Licence /Previous renewal certificate.

2.      Memorandum and Article of Association if it is Ltd. or Pvt. Ltd.

3.      Non-conviction affidavit of proprietors/partners/directors.

4.      Non- conviction affidavits of all the technical staff.

5.      Attested photocopies of qualification and approval documents of all the technical staff.

6.      List of Directors in Form 32.

7.      Director/person responsible for day to day work and conduct of the business of the firm.

8.      Non-conviction affidavit of authorised signatory.

Stage 2


The applicant will upload and submit the application completed in all respect to the State Drugs Controller alongwith all relevant documents. Application is scrutinized and the application is forwarded to the CDSCO(NZ), Ghaziabad for joint inspection of the premises of the applicant firm.
